Yes. The code contains the number and text, and the message goes over the mobile network. That is its main advantage over a URL code in places with no data coverage.

Is the message sent automatically when scanned?

No. The messaging app opens with everything filled in, and the user presses send. Automatic sending is not possible on iOS or Android, by design.

Do standard message rates apply?
